Frequently Asked Vision Care Questions
1. Q. Do my benefits include vision care?
A. Yes, eligible members of UFCW Local 464A who are employed either fulltime or part-time may utilize the vision care Plan.
2. Q. Are my dependents covered?
A. The dependents of eligible UFCW Local 464A members who are employed fulltime are provided with vision care benefits.
3. Q. What about dependents of Part-Time Members?
A. The dependents of members who are employed part-time do not receive vision care coverage.
4. Q. I am fulltime, when does the vision coverage end for my dependent children?
A. Coverage for children ends when they have reached age 19. If a child is attending an accredited four (4) year college (for an academic degree) on a full-time basis, coverage under the Plan remains in effect until he/she leaves school or reaches age 23 whichever comes first. To maintain coverage for full-time students between age 19 and 23 a letter must be sent to the attention of the Benefits Department at the Little Falls, NJ office prior to the start of each semester (fall and spring) stating that the child is a full-time student.

6. How do I obtain my Group Reimbursement Welfare Plan booklet?
A. See your Shop Steward and ask him/her to obtain a copy for you from your Local 464A Union Agent or Service Representative.
7. Q. Can I get vision care anywhere I want?
A. No, the vision care benefit provided by UFCW Local 464A is available only at participating vision care centers. See your Group Reimbursement Welfare Plan booklet or click here for a list of the centers.
8. Q. Can I use the vision care plan anytime I wish?
A. Members and their eligible dependents are permitted to visit a participating vision care center once every two years for a visual examination and either a pair of eyeglasses or contact lenses.
8. Q. Are sunglasses included under the vision care plan?
